Note : Before you start filling this form please keep a scanned copy of following documents ready.
If we find any Incomplete information, you might end up with a rejected application
1) ( * ) marked fields are mandatory.
2) Most recent high school or college transcript.
3) Three reference letters from non relatives that help the scholarship committee to have a better understanding of the type of person that you are. These letters maybe uploaded in your application file or can be emailed directly to lpsofusafoundation@gmail.com
4) Most recent SAT, ACT, GRE or Equivalent Score Report.
5) Letter of Admission to College.
6) Your current high resolution picture.
7) First 2 pages of your parents most recent IRS 1040 Tax Return.
